What criteria are used to decide between All-on-4 and All-on-6?
Which method, All-on-4 or All-on-6, is suitable is determined after checking the amount and density of the jawbone and the bone quality through CBCT. If the jawbone is relatively healthy and sufficient support can be secured with just four implants, All-on-4 may be considered. If the bone condition is somewhat weak or it is necessary to distribute the load on the prosthesis more stably, All-on-6, which places six implants, may be considered. A greater number of implants does not necessarily mean better results, and it is important to find the placement and number that suit the individual’s oral structure. Therefore, it is most accurate to determine which method is suitable for you through a detailed examination and consultation.
Is it okay to have tooth extraction and implant placement on the same day?
Performing tooth extraction and implant placement on the same day can help reduce the overall treatment period and the number of visits, as there is no need to wait for the bone to heal after extraction. However, it is not possible in all cases. If there is acute inflammation at the extraction site or there is not enough remaining bone, it may be more stable to wait for a certain period of healing after extraction before placing the implant. For this reason, the bone condition and presence of inflammation at the extraction site are first checked with CBCT, and same-day extraction and implant placement are performed only when the conditions are met. It is most accurate to determine whether your case qualifies through a detailed examination.
Can a temporary prosthesis be placed immediately on the same day as the implant placement?
One of the features of All-on-4/6 is that, when the conditions are met, a temporary prosthesis can be placed on the same day as implant placement. This can help maintain a certain level of chewing function and aesthetics during the recovery period. However, this is only possible when the placed implants have sufficient initial stability. If the bone condition is weak or the initial stability is considered insufficient, it may be more stable to proceed with the final prosthesis after allowing a certain period of time. Whether a temporary prosthesis is placed and when it is placed are determined by comprehensively considering the bone condition and implant stability at the time of placement.
Can the procedure still be performed even if the jawbone has been significantly resorbed due to long-term denture use?
Even when the jawbone has been significantly resorbed due to long-term denture use, treatment may still be possible by first reinforcing the insufficient bone through procedures such as sinus lift or bone grafting, and then placing the implants in stages. In cases with severe bone resorption, the treatment plan can be more complex than in typical cases, and the overall treatment period is often longer when the healing period after bone grafting is included. However, having significantly insufficient bone does not necessarily mean that treatment is impossible. After checking the amount of remaining bone and the extent to which grafting is possible through a detailed CBCT examination, a step-by-step treatment plan is established. Especially in difficult cases like these, having multiple medical professionals diagnose and discuss the case together can help improve the stability of the treatment.
Do All-on-4/6 implants also require regular maintenance?
Yes, even after the final prosthesis is placed, regular checkups are necessary to ensure that the bone and implants remain stably integrated and that the occlusion of the prosthesis is appropriate. Since All-on-4/6 has a structure in which a small number of implants support the entire prosthesis, a problem with an individual implant can affect overall chewing function, making regular checkups even more important. In particular, for cases involving bone grafting or severe jawbone resorption, monitoring the progress during the initial recovery period is even more important. The recommended maintenance intervals and items to be checked may vary depending on the individual’s implant placement method and oral condition.
